What is included in a Medicare wellness exam?

Medicare Wellness Exams are a specialized preventive service covered by Medicare to help you stay healthy and detect health risks early.

A Medicare Wellness Exam at Lifeline Primary Care isn’t a traditional physical, but rather a comprehensive review focused on your overall wellness and personalized prevention. During your visit, our caring healthcare providers will:

Review Your Medical History: We’ll discuss your personal and family health history to identify any risks of conditions such as heart disease, diabetes, or cancer.

Check Vital Signs & Measurements: Our team will record your height, weight, blood pressure, and other key measurements to monitor your health.

Review Medications & Care Team: We’ll go over all your current medications, supplements, and any specialists you see to ensure safe and coordinated care.

Screen for Mood & Cognitive Health: As part of our commitment to whole-person care, we’ll screen for depression, memory loss, dementia, and assess your ability to live safely and independently at home.

Lifestyle & Wellness Counseling: We’ll talk with you about your diet, exercise, and use of tobacco or alcohol, providing counseling and referrals to help you reach your health goals.

Update Preventive Screenings & Vaccines: Our providers will make sure you’re up to date on important screenings and immunizations, such as mammograms, colonoscopies, or flu shots.

After your exam, you’ll receive a personalized prevention plan tailored to your needs, designed to help you maintain your health, manage chronic conditions, and prevent diseases.

How does a Medicare wellness exam help support preventive care?

Medicare wellness exam is an essential part of our commitment to preventive healthcare. This annual exam provides our patients with a dedicated time to partner with our medical team to focus on their long-term health and well-being. Unlike visits that address specific illnesses or immediate concerns, the Medicare wellness exam at Lifeline Primary Care is designed to take a comprehensive look at your overall health, your medical history, and your risk factors for various conditions.

During your wellness exam, our providers will measure your vital signs—including height, weight, and blood pressure—and thoroughly review your immunization records. We’ll work with you to develop or update a personalized prevention plan, outlining recommended screenings, vaccinations, and healthy lifestyle changes that are tailored to your unique needs and risk profile.

At Lifeline Primary Care, we believe that prevention is key to maintaining good health as you age. That’s why our Medicare wellness exams include important screenings for cognitive impairment and depression, which are especially relevant for older adults. Identifying these concerns early allows us to address them proactively, helping you maintain a high quality of life and reducing the risk of more serious health issues or hospitalizations in the future.

By scheduling your Medicare wellness exam, you’re taking an important step in managing your health. Our providers are dedicated to building a lasting partnership with you, empowering you to stay up to date on preventive services such as cancer screenings, diabetes checks, and heart health assessments. Together, we can create a solid foundation for healthier aging and a brighter future.

Who is eligible for a Medicare wellness exam?

If you are enrolled in Medicare Part B, you may be eligible for important preventive services designed to support your long-term wellness.

Welcome to Medicare Preventive Visit:
New Medicare Part B patients can schedule a “Welcome to Medicare” visit at Lifeline Primary Care within the first 12 months of their coverage. This one-time appointment gives you and your provider an opportunity to review your medical history, assess your current health, and discuss preventive care that’s right for you.

Annual Wellness Visit:
After your first year with Medicare Part B, you can schedule an Annual Wellness Visit at Lifeline Primary Care once every 12 months. Please note, this is not a traditional physical exam. Instead, we focus on creating or updating your personalized prevention plan, reviewing your risk factors, and making sure you’re up-to-date with recommended screenings and immunizations.

To qualify for the Annual Wellness Visit, you must not have received another Medicare wellness exam or similar preventive service in the last 12 months. Both the Welcome to Medicare visit and the Annual Wellness Visit are available to all Medicare Part B beneficiaries, regardless of age, as long as the timing requirements are met.

What should you bring to your Medicare wellness exam appointment?

At Lifeline Primary Care, we want to ensure you get the most out of your Medicare wellness exam. To help us provide the best possible care, please bring the following items to your appointment:

Your Medicare Card and Insurance Information: Bring your Medicare card along with any supplemental insurance cards so our team can accurately bill your visit.

A Complete List of Medications: Include all prescription medications, over-the-counter drugs, vitamins, and supplements you currently take. If possible, note the dosage and how often you take each item. This helps your provider review your medications and avoid any interactions.

Medical History Information: Please bring details about your personal medical history as well as any significant family history, such as heart disease, diabetes, or cancer. This information allows us to assess your risk factors and create a personalized care plan.

List of Healthcare Providers: If you see any specialists or other healthcare professionals, bring their names and contact information. This helps us coordinate your care and ensure everyone involved in your health is on the same page.

Recent Medical Records: If you’ve had any tests, procedures, or hospitalizations since your last visit to Lifeline Primary Care, bring copies of those records or be ready to share the details.

Health Concerns and Questions: Write down any symptoms, health concerns, or questions you’d like to discuss with your provider during your appointment. This ensures we address all your needs during your visit.

Medical Devices: If you use medical devices such as hearing aids, glasses, or mobility aids, please bring them with you.

Support Person: Consider bringing a trusted friend or family member if you feel you may need help remembering information or communicating your concerns.

At Lifeline Primary Care, our goal is to partner with you in maintaining your health and wellness. Being well-prepared for your Medicare wellness exam helps us understand your health needs and deliver the highest quality care.
